All of our registration materials are contained in this one download. You can open it with the FREE Acrobat viewer, and either:
- Fill it out on your computer and print it (Great for those who like to type).
- Or print it and then fill it out by hand (Perfect for those who like to write legibly).
Note: Payment can be sent in with your forms, or you can go to your choir's page and use the easy PayPal payment buttons to pay online.
IMPORTANT: Be sure to add a note during checkout with your choristers name and include a copy of your PayPal confirmation/receipt with your registration packet.

Audition Downloads:
Singers interested in auditioning are asked to bring a completed Student Audition Form, a letter of support from your parent or guardian ,and we also encourage you to bring a Teacher Recommendation Form from your music teacher to the audition.
